Please send me copies of the original application for a business license/ occupational tax certificate for Literacy Action Inc. , 100 Edgewood Ave NE #600, Atlanta, GA 30303 and the application and E-Verify and SAVE affidavits required by OCGA 36-60-6 and OCGA 50-36-1, respectively, for renewal of the business license occupational tax certificate/ for Literary Action Inc. for the years 2012, 2013, 2014, 2015 and 2016.
